Can I use psilocybin with insulin, Forxiga, Levemir, metformin, and others?

Short answer: I am not aware of well documented direct interactions between psilocybin and insulin (including Levemir), metformin, dapagliflozin/Forxiga, pantoprazole, rosuvastatin, or low-dose aspirin. Still, a session can change appetite, food intake, and activity, so glucose may swing; plan monitoring. With SGLT2s like dapagliflozin, keep up fluids to reduce dehydration risk. Psilocybin is reported to temporarily raise heart rate and blood pressure.

Mounjaro (tirzepatide) can slow gastric emptying, so onset of psilocybin may be delayed and nausea may be stronger. Vortioxetine (Brintellix) may blunt psychedelic effects; serotonin syndrome has been speculated but well documented cases with this combo are scarce. Lorazepam often reduces or aborts the experience. Do not change or pause any medication without your prescriber.

Methotrexate: data on co-use with psilocybin are limited, and I cannot verify safety for this combination. Given multiple medications and possible underlying conditions, discuss suitability and safeguards with your treating clinician, including a glucose plan and cardiovascular review.
